If you’re hoping to lose weight, understanding your metabolism can help. “‘Metabolism’ is really a catch-all word for the different processes going on in the body,” Shawn Talbott, Ph.D., a nutritional biochemist, tells SELF. But when it comes to weight loss, most people are talking about energy metabolism, or how your body burns calories, he explains.

One such lifestyle is the Pritikin Program of diet and exercise. The most expensive but highly regarded test is the direct calorimetry test. A direct calorimeter is a large insulated, air-tight chamber. During a test you spend at least an hour inside the chamber with minimal movement. During that time your released body heat (including expired carbon dioxide and vapors) is measured. Based on these measurements, a resting metabolic rate is calculated. In most situations, this test is not practical due to the expensive equipment needed and the time you need to spend laying in the chamber.
These studies were approved by external Research Ethics Committees (London Queen's Square: 14/LO/0288 and South West Frenchay: 15/SW/0244) and were conducted in accordance with the Declaration of Helsinki (2008). Studies took place at the University of Oxford between September 2014 and September 2016. Participants were healthy, aged 21–57, non-smokers and had no history of major illness. Female participants were using oral contraception to minimize the effects of menstrual phase on results. Participants provided written informed consent prior to inclusion, and completed a confidential medical screening questionnaire to determine eligibility. Anthropometric characteristics are shown in Table ​Table1.1. Sample sizes were chosen following an estimated power calculation based on the effect size in previous work using KE drinks (Clarke et al., 2012b; Shivva et al., 2016).

"If you go at dieting very vigorously your metabolism falls, so it means you lose less weight than the calories you cut," says Susan B. I was super stoked to see that my goodies arrived today at work so tried the Vanilla in my morning coffee with unsweetened almond milk and heavy whipped cream as usual. VLDL apoB-100, LDL apoB-100, and HDL apoA-I were isolated from plasma by sequential ultracentrifugation. The procedures for isopropanol precipitation, delipidation, hydrolysis, and derivatization of apoB-100 were described previously (3). ApoA-I was isolated from the HDL fraction by SDS-PAGE and blotted onto a polyvinylidene difluoride membrane; apoA-I bands were excised from the polyvinylidene difluoride membrane, hydrolyzed overnight (6 M hydrochloric acid, 110°C), and dried for derivatization. Isotopic enrichment of apoB-100 and apoA-I was determined using negative chemical ionization by gas chromatography–mass spectrometry.
Subjects entered a randomized, controlled dietary intervention study. After weight stabilization for 4 weeks, they were randomly assigned to either a hypocaloric diet for 14 weeks immediately followed by a 2-week weight stabilization period or to weight maintenance with consumption of an isocaloric diet for 16 weeks. All tests were performed, at baseline and after 16 weeks, when subjects were at a stable body weight. Body weight, height, waist circumference, and blood pressure were recorded. Body composition was estimated using a Holtain Body Composition Analyser (Holtain, Dyfed, U.K.) from which total fat mass and fat-free mass (FFM) were derived (6). Subcutaneous abdominal adipose tissue and visceral adipose tissue volumes and masses were estimated after magnetic resonance imaging, as described previously (16). All subjects were studied after a 14-h fast. Venous blood was collected for biochemical measurements before stable isotope infusion. LDL apoB-100 and HDL apoA-I kinetics were measured using primed (1 mg/kg), constant (1 mg · kg−1 · h−1) intravenous infusion of [1-13C]leucine (99.5% enrichment; Tracer Technologies, Somerville, MA) for 10 h (6). Blood samples for lipoprotein kinetic estimates were collected before and after isotope injection at 0.5, 1, 1.5, 2, 2.5, 3, 4, 5, 6, 7, 8, 9, and 10 h. Subjects were studied in a semirecumbent position and allowed water only.
To determine the reason for the differences in blood d-βHB concentration, the KE and KS drinks were analyzed for enantiomeric purity. The KE contained >99% of the d-isoform, whereas ~50% of the KS βHB was the l-isoform (Figure ​(Figure1D).1D). Plasma samples from participants who consumed the high dose KS drink (n = 5) were analyzed to reveal higher l-βHB than d-βHB, the total βHB Cmax being 3.4 ± 0.2 mM (Figure ​(Figure1E),1E), with a total βHB AUC of 549 ± 19 mmol.min. After 4 h, plasma l-βHB remained elevated at 1.9 ± 0.2 mM; differences in urinary excretion of the two isoforms could not explain this observation as both d- and l-βHB were excreted in proportion to their blood AUCs (Figure ​(Figure1F).1F). Therefore, in order to determine the time required for l-βHB elimination, a follow-up experiment was undertaken in which subjects (n = 5) consumed 3.2 mmol.kg−1 of βHB as KE and KS with hourly blood and breath sample collection up to 4 h, plus additional samples at 8 h and 24 h post-drink. l-βHB was found to be 1.1 ± 0.1 mM at 4 h, and 0.7 ± 0.2 mM after 8 h, but undetectable after 24 h (Figure 1G). Low amounts of d-βHB (0.3 ± 0.1 mM) were present at 24 h, presumably due to endogenous production. Both ketone drinks significantly increased breath acetone concentration, but at a slower rate than blood d-βHB, reaching a peak after 3 h that was twice as high following the KE (87 ± 9 ppm) than the KS (44 ± 10 ppm), suggesting that d-βHB was readily converted to acetone, but l-βHB was not (p < 0.005, Figure ​Figure1H1H).
Participants refrained from alcohol and caffeine for 24 h prior to each visit AND were asked to consume a similar meal the night before each visit. All studies were carried out at the University of Oxford Human Physiology Laboratories and started at 0800 h following an overnight (>8 h) fast, with a minimum of 72 h between visits. Visit order was randomized prior to commencement by an administrative investigator using a pseudo-random number generator to produce a list of combinations of visit order, which were then allocated based on order of enrolment by a different investigator.
