Some studies have shown as few as 3% of people with NASH developing cirrhosis, while others have shown as many as 26% doing so. There's no test or risk factor that predicts who will develop cirrhosis and who won't, although one study did find that people who are older or whose initial liver biopsies showed more inflammation were at greater risk. It's clear, though, that the prognosis for NASH is far better than it is for steatohepatitis that's the result of heavy alcohol consumption. Perhaps as many as half of all those with alcoholic steatohepatitis (which lacks a handy acronym) go on to develop cirrhosis.

An animal study seeking to document molecular mechanisms showed that three aerobic sessions per week were adequate to reduce weight gain, shrink belly fat, and clear out liver fat. In humans who lost weight and then exercised 40 minutes twice a week, even if they regained a slight amount of weight, they did not regain “dangerous fat” that is associated with fatty liver. I would always suggest more exercise than this study. However, the study shows that keeping up even a moderate exercise program can prevent sliding back into the fatty liver problem.


“Muscle can really drive your metabolic rate,” says Talbott. That’s because muscle is more metabolically active than something like fat or bone, meaning it expends more calories while performing its duties. Luckily, the best way to gain muscle can also make you feel like a total badass. Whether you’re using weights or only doing bodyweight exercises, strength training can add some more muscle to your frame. And don’t worry that doing these types of exercises will make you more muscular than you’d like. It’s actually really hard for women to gain a ton of muscle, and it would take a lot more effort than regularly incorporating strength training into your workout schedule, says Talbott.

Ketosis means that your body is in a state where it doesn't have enough glucose available to use as energy, so it switches into a state where molecules called ketones are generated during fat metabolism. Ketones can be used for energy. A special property of ketones is that they can be used instead of glucose for most of the energy needed in the brain, where fatty acids can't be used. Also, some tissues of the body prefer using ketones, in that they will use them when available (for example, the heart muscle will use one ketone in particular for fuel when possible).
The next thought was that lowering dietary fat, especially saturated fats, may help lower cholesterol. While untrue, there are still many who believe it. In the 1960’s the Framingham Diet Study was set up to specifically look for a connection between dietary fat and cholesterol. This was the same Framingham as the famous Heart Studies, but references to the Framingham Diet study are virtually non-existent. Why haven’t you heard of it, before? Well, the findings of this study showed no correlation between dietary fat and cholesterol whatsoever. Because these results clashed with the prevailing ‘wisdom’ of the time, they were suppressed and never published in a journal. Results were tabulated and put away in a dusty corner. Dr. Michael Eades was able to track down a copy of this forgotten gem and wrote about it’s eerily prescient findings here.

You've no doubt heard that sugar has been the number one culprit behind weight gain. In fact, over the last few years, sugar has all but been demonized as the terrible-for-you ingredient that is as addictive as cocaine and will lead to heart disease and diabetes. And while that's all a bit of an exaggeration, there's no denying that sugar, especially added sugar found in packaged food and sweets, isn't great for your health.

Carbohydrate: Most of what determines how ketogenic a diet is will depend on how much carbohydrate is eaten, as well the individual's metabolism and activity level. A diet of less than 50 or 60 grams of net (effective) carbohydrate per day is generally ketogenic. Some sources say to consume no more than 20 grams of carbohydrates per day, while others cite up to 50 grams, and many recommend no more than 5 percent of calories from carbs. However, athletes and people with healthy metabolisms may be able to eat 100 or more grams of net carbohydrate in a day and maintain a desired level of ketosis. At the same time, an older sedentary person with Type 2 diabetes may have to eat less than 30 net grams to achieve the same level.
The effects of ketone drinks on endogenous insulin secretion are unclear. Whilst the small increase in plasma insulin after KE and KS drinks may have been due to the small quantity of dextrose in the diluent, it has been proposed that ketones could potentiate or even stimulate insulin secretion. Isolated pancreatic islets secreted insulin when stimulated by ketones at glucose concentrations of >5 mM (Biden and Taylor, 1983), and small amounts of insulin are secreted in vivo following exposure to exogenous ketones in animals (Madison et al., 1964; Miles et al., 1981). In response to an intra-venous 10 mM glucose clamp, ketone ester drinks increased glucose uptake and plasma insulin (Holdsworth et al., 2017). The increases in insulin with ketone drinks taken whilst fasted were small compared to the increases seen when the ketone ester drink was consumed with a meal and with consumption of a dextrose drink. Furthermore, the lack of difference in peak plasma insulin between the two latter conditions indicates that nutritional ketosis did not inhibit or increase normal carbohydrate induced insulin production.
The buildup of fat in the liver can result in inflammation and scarring (fibrosis). This more serious form of NAFLD — called nonalcoholic steatohepatitis, or NASH — can cause severe liver damage and eventually lead to liver failure or liver cancer in a small percentage of people. Although rare, the scarring can also harden the liver and impair its ability to function properly — a condition known as cirrhosis.

Ingestion of ketone drinks significantly decreased overall mean plasma FFA from 0.7 to 0.4 mM, TG from 1.1 to 0.9 mM and glucose from 5.7 to 4.8 mM after 1 h (all p < 0.05). Concentrations were the same as at baseline by 4 h, with FFA at 0.6 mM, TG at 0.9 mM and glucose 5.1 mM (Figures 2A–C). There was a rise in insulin concentrations 30 min following all drinks, probably due to the small amount of carbohydrate in the sweetener (Figure ​(Figure2D2D).
Tracer-to-tracee ratios were modeled using SAAM-II (University of Washington, Seattle, WA) from which fractional catabolic rates (FCRs) of LDL apoB-100 and HDL apoA-I were estimated from the best fit of the model to the data. The apoB-100 model consisted of seven compartments (20). Compartment 1 represents the input of the tracer, which is connected to an intrahepatic compartment (compartment 2) that accounts for synthesis and secretion of apoB-100 into the VLDL pool (compartment 3). Compartments 3 and 4 account for the kinetics of apoB-100 in the VLDL fraction. Compartments 5 and 6 account for the kinetics of apoB-100 in the intermediate-density lipoprotein (IDL) and LDL fractions, respectively. The apoA-I multicompartmental model consisted of three compartments (21). Compartment 1 represents the tracer input, which is incorporated into an intrahepatic compartment (compartment 2) that accounts for the synthesis and secretion of apoA-I into the HDL fraction (compartment 3). LDL apoB-100 and HDL apoA-I transport rates were calculated by multiplying the FCR by pool size (milligram per kilogram of FFM per day).
