In an earlier study, we demonstrated that in the short term, weight loss reduced hepatic steatosis and fibrosis in patients with chronic HCV.11 In the current study, we demonstrated a similar early histological improvement in an additional small number of patients with obesity related fatty liver disease. Although liver biopsies were not performed at 15 months, it is likely that the sustained improvement in ALT and fasting insulin in patients who maintained weight loss was accompanied by a sustained reduction in hepatic steatosis and necroinflammatory activity. With long term weight maintenance there is likely to be an even greater resolution of hepatic fibrosis.
The reason these minute-to-minute measurements are so important is that they allow the chamber to detect subtle shifts of energy expenditure — as little as a 1.5 to 2 percent change over 24 hours — in a way no other tool can. “If you have an intervention — a drug or diet — that changes a person’s physiology by a small percentage, we can measure that,” Chen said proudly.

Insulin resistance did not impact on a patient’s ability to decrease weight or waist circumference during the initial three month period. However, the ability to maintain this weight loss for 12 months was significantly associated with insulin resistance. Weight maintainers had significantly higher fasting insulin levels (p = 0.03) and HOMA (p = 0.02) at t = 0 than those patients who regained weight. There was a significant negative correlation between the amount of weight regained during follow up and metabolic factors associated with insulin resistance such as fasting insulin (rs = −0.47, p = 0.01), glucose (rs = −0.40, p = 0.03), and HOMA (rs = −0.54, p = 0.002).
Plus, in the last 30 years, exercise has stayed about the same, while overweight and obesity have skyrocketed. So something else must be at play – like the type of food we’re eating. That part has gotten steadily worse over the years, as highly-processed sugary foods and sodas have taken over as our go-to choices. “According to the Lancet global burden of disease reports,” they write, “poor diet now generates more disease than physical inactivity, alcohol and smoking combined.” This is a disturbing statistic. But it gets worse.

But monitoring ketone levels in urine is especially important for people with diabetes when they are ill, stressed, or have persistently high blood glucose levels. A positive test result for urine ketones in people with diabetes may be a warning sign for ketoacidosis – a serious complication of diabetes (primarily type 1) associated with extremely high levels of ketones in the blood. If left untreated, diabetic ketoacidosis can lead to coma and death.
Now my Dr. wants me to go to an internest and possibly get on meds which I am very opposed to doing. Could my weight loss diet have caused this? Also, I take Evening Primrose oil capsules, pro biotics, fiber DX fiber chews, MSM and Choelius Forskeli. I added Vitamin D3 2000 IU during these last 3 months at the Dr’s suggestion. But I’d been hearing a lot about the no-sugar craze and some talk about if sugar is really bad for you, and it got me thinking about my diet. The truth is: I have an insane sweet tooth. I eat ice cream every day. I even held a taste test at Runner’s World once. So if anyone could stand to cut back on sugar, I figured it was me. I gave myself 30 days to see what would happen. But it wasn’t all or nothing—I made a few guidelines:
And recently, a six-year study involving 18,000 people with heart disease affirmed that for reducing LDL levels, the lower, the better. The study was reported at the annual meeting of the American Heart Association.3 Half the subjects lowered their LDL, on average, to 69; the other half reduced LDL to 54. Both groups were rewarded with few heart events over the six-year period, but the group with the lower LDL, 54, ended up the winner. It had 6.4% fewer events – heart attacks, heart disease deaths, strokes, bypass surgeries, stent procedures, and hospitalizations for severe chest pains – than the group with the higher LDL.

At each meal, focus on building a healthy plate that includes quality, lean protein, like poultry and fish, a moderate amount of healthy fats, like avocado and olive oil, and foods that have naturally occurring fiber, like green, leafy vegetables and whole grains. Aim for foods that have 3 grams of fiber or more per serving. “All of that helps slow down the rate at which your body breaks down [carbs] and uses it for energy,” Lemond explains. “Focus on what to put on your plate instead of what to leave off your plate.”
The American Heart Association recommends no more than six teaspoons (25 grams) of added sugar a day for women and nine teaspoons (36) grams for men. "Added sugars contribute zero nutrients but many added calories that can lead to extra pounds or even obesity, thereby reducing heart health," the AHA wrote on its website. And the World Health Organization (WHO) recommends only 10 percent of your diet come from added sugar, noting that "a further reduction to below five percent or roughly 25 grams (6 teaspoons) per day would provide additional health benefits."
The next thought was that lowering dietary fat, especially saturated fats, may help lower cholesterol. While untrue, there are still many who believe it. In the 1960’s the Framingham Diet Study was set up to specifically look for a connection between dietary fat and cholesterol. This was the same Framingham as the famous Heart Studies, but references to the Framingham Diet study are virtually non-existent. Why haven’t you heard of it, before? Well, the findings of this study showed no correlation between dietary fat and cholesterol whatsoever. Because these results clashed with the prevailing ‘wisdom’ of the time, they were suppressed and never published in a journal. Results were tabulated and put away in a dusty corner. Dr. Michael Eades was able to track down a copy of this forgotten gem and wrote about it’s eerily prescient findings here.
But before I gave up the sweet stuff, I questioned what it would do to my body-would I crave it more than usual? Is there such a thing as a sugar detox? "There are many theories on sugar and addiction, but I don't think there's any concrete evidence proving that a person can be addicted to sugar," says Marie Spano, R.D. and sports nutritionist for the Atlanta Hawks. She thinks the habitual intake and oh-so-good taste are actually what make it difficult to kick a sugar habit (see: The Science Behind Your Sweet Tooth). No one said this was going to be easy!
In addition to liver problems, people with fatty liver disease and NASH need to be more worried about heart disease and stroke. Their risk of dying from cardiovascular disease is twice as high as people that don't have NASH. One reason may be related to the inflammatory and other factors pumped out by a fat-afflicted liver cells that promote damage to the insides of arteries and make blood more likely to clot, a combination that can lead to heart attack or stroke.
One such lifestyle is the Pritikin Program of diet and exercise. Normally, fatty acids are oxidized to become energy with the help of some hormones, but insulin prevents lipolysis and hampers the breakdown of adipose tissue. This results in free fatty acids being released into the bloodstream which leads to an accumulation of triglycerides. Higher levels of triglycerides are also associated with higher levels of LDL-cholesterol and lower levels of HDL-cholesterol (12).
Over four visits, participants (n = 15) consumed 1.6 and 3.2 mmol.kg−1 of βHB as KE (141 mg/kg and 282 mg/kg of R-3-hydroxybutyl-R-1,3-hydroxybutyrate) or as KS (KetoForce, KetoSports, USA) sodium and potassium βHB, containing 1.6–3.2 g of each cation), plus 6 g of sweetener containing 19 kCal (4 g of carbohydrate) (Symrise, Holzminden, Germany), diluted to 300 ml using water. Drink blinding was not possible due to unmaskable differences in taste (bitter vs. salty).

To understand why the liver is the focus of a diet that promises to “reset” your metabolism, it’s important to understand a few basic facts about what it does. The liver is involved in virtually every metabolic process in the body, including turning nutrients from food into substances your body can use, breaking down fats, storing sugar as triglycerides, creating energy and, perhaps most important of all, getting rid of toxic substances. The liver is so important, there’s even a direct route from the digestive organs to the liver called the portal vein. An adult liver weighs just over three pounds and is one of the largest organs in the human body.
Reduce your intake of calories from added sugar even more by reading food labels. This will help you identify foods that contain added sugars but aren't sweet, obvious sources. For example, many condiments, such as salad dressing, ketchup and barbecue sauce, contain added sugar. Restricting these items in your diet can help you further reduce how many calories you eat each day. Examine the ingredient list to determine if the food contains sugar, and then glance at the nutrition facts to see how many grams of sugar a serving of each food contains. This will help you choose the foods lowest in added sugars.
Focus on protein and fiber. According to Roberts, research is ongoing on the topic of metabolism falling below a normal level. "Perhaps higher-protein diets help prevent the fall," she says. "Also, definitely higher-fiber diets will have a protective effect." She and her colleagues found that when people with stable weights replaced refined grains with whole grains, they were able to modestly increase their BMR (or RMR). That's why a high-fiber diet is the cornerstone of her weight-loss program. Other studies confirm that eating foods high in protein and fiber and lower on the glycemic index lead to less hunger and greater levels of fullness, which help combat the increased hunger caused after weight loss. Aim to eat at least 25 to 35 grams of fiber per day.
Resting Metabolic Rate (RMR): The amount of calories burned while in a resting/quiet state. RMR for an average person is the largest part of total metabolism accounting for 65-75% of calories burned daily. We have little control over RMR unless we add a significant amount of muscle or weight leading to an increase of calories burned (3-6cals/day/pound depending on muscle to fat content).
Other medications that may cause liver inflammation, most of which will resolve when the medication is stopped. These include antibiotics such as nitrofurantoin (Macrodantin, Furadantin, Macrobid), amoxicillin and clavulanic acid (Augmentin, Augmentin XR), tetracycline (Sumycin), and isoniazid (INH, Nydrazid, Laniazid). Methotrexate (Rheumatrex, Trexall), a drug used to treat autoimmune disorders and cancers, has a variety of side effects including liver inflammation that can lead to cirrhosis. Disulfiram (Antabuse) is used to treat alcoholics and can cause liver inflammation.
If your physician performs a urine test and finds your ketones to be high, it's important to notify him you are losing weight, according to the Joslin Diabetes Center. He may recommend an additional blood test to ensure your blood-glucose levels are not high — which can be a sign of diabetes. However, dieters with high ketone levels should not experience high blood-glucose levels.

Over five visits, participants (n = 16) consumed either 4.4 mmol.kg−1 of βHB (2.2 mmol.kg−1 or 395 mg/kg of KE; 1 mole of KE delivered 2 moles of d-βHB equivalents): twice whilst fasted, and twice following a standardized meal, or an isocaloric dextrose drink without a meal. To improve palatability, drinks were diluted to 500 ml with a commercially available, citrus flavored drink containing 65 kCal (5 g of carbohydrate) (Glaceau, UK). The dextrose drink was taste-matched using a bitterness additive (Symrise, Holzminden, Germany). The standard meal consisted of porridge oats (54 g), semi-skimmed milk (360 ml) and banana (120 g), giving 600 kCal per person, with a macronutrient ratio of Carbohydrate: Protein: Fat of 2:1:1.
One of the best ways to cut sugar from your diet is to focus on noshing whole foods instead of packaged, processed foods, like cookies, cake, candy, granola bars, and cereals. Whole foods include fruits, veggies, whole grains, nuts, and seeds. Although your body may by now be primed to crave sugar, the more whole foods you eat, the more you’ll come to enjoy them. “Your taste buds will adapt,” Lemond says.

The salts typically utilize sodium, potassium, calcium, or magnesium as the cation. Because these cations vary in molecular weight and valence (1+ or 2+), the amount of mineral delivered per gram of BOHB varies from 10% for the magnesium salt to 27% for potassium. Given that recommended daily intakes of these various minerals range from a few hundred milligrams up to 5 grams, whereas the daily ketone intake goal to mimic nutritional ketosis blood levels would need to be on the order of 50 grams, achieving this goal with ketone salts would severely challenge human dietary mineral tolerance.
My case was a quandary: I already exercised diligently, running about four miles most days. I had long ago given up red meat and most cheese. Yet my bad cholesterol last October was 169, way above my recommended high of 130 and an optimal 100. (People’s LDL goals depend on their number of risk factors such as smoking, diabetes or high blood pressure.)
Exercise diaries were not available for two patients. Before commencement of the programme only 10 patients (29%) were involved in any form of regular exercise (range 40–350 min/week). From t = 0 to t = 3 months, all patients except two (6%) commenced regular weekly aerobic activity with mean exercise time of 214 (166) minutes per week (range 0−840 min/week). During the 12 month weight maintenance programme, overall exercise decreased to a mean of 120 (140) min/week (range 0–560 min/week). In those patients who maintained weight, exercise levels were sustained at recommended levels of 150 (160) min/week whereas those that regained weight had reduced their level of exercise to 50 (54) min/week (p = 0.02).
Solutions – switch to organic food as much as possible, avoid all chemical sprays and commercial household cleaning products, use organic cosmetics, shampoos and body lotions. Embark on a supervised, 4-6 week detoxification program at least twice a year (beginning of spring and autumn are good times) when you can slowly and safely excrete the toxins over time.
Twenty seven patients had hepatic steatosis in association with chronic hepatitis C and 11 of these patients were included in an earlier report describing the initial response to a three month weight reduction programme.11 Data from these patients were included in this study to monitor longer term outcomes. All patients with chronic HCV were ineligible for, or non-responders to, current antiviral treatment. Sixteen patients with non-HCV obesity related steatosis were included. Ten of these 16 patients had a clinical and histological diagnosis of NAFLD. In another four non-HCV patients, steatosis was present in association with another non-steatogenic primary liver disease (inactive hepatitis B, n = 2; nitrofurantoin induced autoimmune chronic active hepatitis, n = 1; and primary biliary cirrhosis, n = 1). In the remaining two patients, one had been treated for pituitary Cushing’s disease and another had previously received tamoxifen therapy for breast cancer which had been ceased for six months. This patient cohort reflects the increasingly common finding of overweight and steatosis in association with another liver disease. When steatosis was present with hepatocyte ballooning, Mallory’s hyaline or subsinusoidal fibrosis in the acini, patients were subclassified as non-alcoholic steatohepatitis (NASH).17
Except for gallstone disease and some viral infections such as hepatitis A, C, and infectious mononucleosis, most liver diseases are managed and not cured. Liver disease can progress to cirrhosis and liver failure. Associated complications may include increased risk of bleeding and infection, malnutrition and weight loss, and decreased cognitive function. Some liver diseases are associated with an increased risk of developing liver cancer.
Liver biopsy specimens were fixed in 10% neutral buffered formalin and embedded in paraffin. Steatosis was graded as follows: 0 (<5% of hepatocytes affected); 1 (5–30% of hepatocytes affected); 2 (30–70% of hepatocytes affected); or 3 (>70% of hepatocytes affected). The degree of inflammation and fibrosis was assessed and graded using the modified Knodell scoring system of Ishak.21 Sections were analysed by an experienced hepatopathologist (AC) who was blinded to patient identity and time of biopsy.

Fatty liver disease occurs when some of those fat molecules accumulate inside liver cells. The presence of those fattened cells can then lead to inflammation in the liver and damage to surrounding liver tissue. Once that happens, if excess alcohol is not involved, the condition is called nonalcoholic steatohepatitis (steato- for fat and –hepatitis because the liver is inflamed). Fortunately, that unwieldy name boils down to a handier acronym, NASH. Estimates vary quite a bit, but it seems that 5% to 10% of people with fatty liver disease go on to develop NASH.
Don’t worry about how much you eat, because you will never be able to control that. Rather, focus on what you eat, the quality of the food you eat, the composition of the food you eat (high in fiber, good quality protein and fat, low in starch and sugar). Then, you won’t be hungry and will shift from fat storage to fat burning. And you will prevent most chronic disease including heart disease, type 2 diabetes, cancer and dementia.
That void has helped PfizerInc.’s Lipitor, the most famous statin, become the world’s top-selling drug with another statin, Merck & Co.’s Zocor, not far behind. Because of all of this activity, your liver may be in need of a little TLC. When it's overworked, toxic residues can build up, causing inflammation that is associated with obesity. A stressed out liver can also cause fat to build up, especially around the belly. Added together, this can mean that no matter how much you restrict calories, weight loss is near impossible-unless you detox your liver.
Meanwhile, the liver begins to burn fatty acids as an alternative energy source, resulting in the accumulation of extremely high levels of ketones in the blood.10 These ketone levels (> 20 mmol/L) can exceed normal fasting levels more than 200 to 300 times.1 Since ketones are mildly acidic, this deluge of ketones causes the blood to become excessively acidic (metabolic acidosis), increasing the risk of coma and death if not timely treated.
The question, therefore, became, what causes high blood levels of cholesterol? The first thought was that high dietary intake of cholesterol would lead to high blood levels. This was disproven decades ago. One might (mistakenly) think that decreasing dietary cholesterol may reduce blood cholesterol levels. However, 80% of the cholesterol in our blood is generated by the liver, so reducing dietary cholesterol is quite unsuccessful. Studies going back to Ancel Key’s original Seven Country Studies show that how much cholesterol we eat has very little to do with how much cholesterol is in the blood. Whatever else he got wrong, he got this right – eating cholesterol does not raise blood cholesterol. Every single study done since the 1960s has shown this fact repeatedly. Eating more cholesterol does not raise blood levels.
The effects of ketone drinks on endogenous insulin secretion are unclear. Whilst the small increase in plasma insulin after KE and KS drinks may have been due to the small quantity of dextrose in the diluent, it has been proposed that ketones could potentiate or even stimulate insulin secretion. Isolated pancreatic islets secreted insulin when stimulated by ketones at glucose concentrations of >5 mM (Biden and Taylor, 1983), and small amounts of insulin are secreted in vivo following exposure to exogenous ketones in animals (Madison et al., 1964; Miles et al., 1981). In response to an intra-venous 10 mM glucose clamp, ketone ester drinks increased glucose uptake and plasma insulin (Holdsworth et al., 2017). The increases in insulin with ketone drinks taken whilst fasted were small compared to the increases seen when the ketone ester drink was consumed with a meal and with consumption of a dextrose drink. Furthermore, the lack of difference in peak plasma insulin between the two latter conditions indicates that nutritional ketosis did not inhibit or increase normal carbohydrate induced insulin production.
For example too much estrogen makes women feel bloated, fatigued, irritable and crave sugar thus putting on weight. Gut symptoms are also very common including food allergies and intolerances, bloating, leaky gut, constipation, liver damage, and others. Importantly, many vegetarians who eat insufficient protein are at risk of detox problems and toxicity.

Sure, food labels will list “sugar” on an ingredient label, but sugar also comes in a number of other forms: glucose, fructose, sucrose, corn syrup, honey, dextrose, to name a few. And often times, food companies will incorporate multiple types of sugars into their products.
