I have hypothyroidism from chemical exposure, I am on natural thyroid and T3. It has been a real struggle, My doctor told me most people in my condition would be severely obese at this stage, but i’m not. I’ve been using perfect keto trying to stay in ketosis, I currently weigh 164 lbs. down from 174. I should weigh 120 but would be content with 135. any suggestions would be great !

Eat a hot cereal for breakfast, such as oatmeal or grits, instead of cold cereal, which is likely to be high in sugar. If you need a sweet start to your day, add a piece of fruit to your breakfast but avoid fruit juice, which is too concentrated in sugar. Avoid instant or flavored hot cereals as they are often loaded with sugar. Instead use plain oatmeal, grits or cream of rice and add some fresh fruit.

Like glucose, ketones are important metabolic fuels reabsorbed through the kidneys as the blood is filtered. When blood passes through the kidneys, small molecules like glucose and ketones end up in the urine, and so must be taken back up. Reabsorption of ketones prevents energy wastage and is especially important during extended fasts. The body doesn’t want to flush good energy down the drain, so ketones appear to be completely reabsorbed from the urine at low blood levels, like after an overnight fast. When the level of ketones in the blood exceeds the reabsorption capacity of the kidneys, the excess ketones spill over into the urine.

I did avoid takeout and restaurants for the 10 days, because it's nearly impossible to know if sugar is added to dishes. This time period included Winter Storm Jonas, so if that doesn't show dedication, nothing will. But I'll fully admit this isn't a sustainable goal-10 days was definitely my max. I missed Indian takeout! To avoid added sugar when eating out, "be very careful about sauces and dressings, including anything ketchup or BBQ based," advises Spano. She suggests asking for sauces and dressing to be served on the side so you control the amount. And choose oil and vinegar for salads instead of heavy sauces to avoid even more sugar.
OK, got it: keep the sugar down for overall health. But what's the sugar threshold for weight loss? Turns out, there's not a one-size-fits-all number. "There isn't a specific value for how many grams of sugar you should eat for weight loss," Ysabel Montemayor, RD, lead dietitian at Fresh n' Lean, told POPSUGAR "Many foods contain sugar, such as breads, grains, beverages, sweets, dairy products, fruits, and even vegetables." She added that the Dietary Guidelines for Americans recommend, like the WHO, no more than 10 percent of calories come from added sugar. So for a 2,000-calorie-a-day diet, this would be around 50 grams of added sugar. Plus carbs, which contain sugar, should constitute 45-65 percent of your diet, according to the Dietary Guidelines for Americans (about 225-325 grams of total carbs).
Those hungry fat cells suck up all the available fuel in your blood stream (glucose, fats, ketones). Your body then thinks, “Oh, my god, I am starving. I better eat more and slow my metabolism, so I don’t die.” The problem is, anything you eat gets sucked up into those fat cells around your belly, leading to a vicious cycle of hunger, overeating, fat storage and a slowing down of your metabolism. No wonder we gain weight and can’t lose it.
Making a homemade granola is just one example. I realized something my friend has been saying forever: It’s best to just make things yourself. I love making cookies, but they’re packed with sugar. So I took one of my favorite recipes and tweaked it to make it a little healthier. "If you go at dieting very vigorously your metabolism falls, so it means you lose less weight than the calories you cut," says Susan B. Roberts, Ph.D., senior scientist at the USDA Human Nutrition Research Center at Tufts and founder of the online iDiet weight-loss program. "Slower dieting has a smaller effect. Once you have lost weight and stabilized, if you have been going at a moderate rate of one to two pounds per week, there does not seem to be a long-term impact. The prevalence of obesity and overweight has risen at an alarming rate over the past 20 years.1 In addition to increasing the risk of the development of type 2 diabetes, hypertension, and dyslipidaemia, excess body weight also has an adverse effect on the liver. Non-alcoholic fatty liver disease (NAFLD) is an increasingly recognised condition that is often seen in patients who are overweight or diabetic or in those with the insulin resistance syndrome.2 It encompasses a wide spectrum of liver pathology, ranging from steatosis to steatohepatitis, fibrosis, and cirrhosis.3 In patients with NAFLD, the clinical risk factors that are associated with liver injury include elevated body mass index (BMI), visceral adiposity (increased waist circumference), presence of type 2 diabetes, and systemic hypertension.2,4,5
The outlook and outcome for a patient depends upon the underlying diagnosis. Interestingly, in patients with cirrhosis, there may be little correlation between the amount of damage found on liver biopsy and the ultimate outcome. A patient may never develop symptoms and have a normal life-span or may develop significant symptoms with seemingly minimal disease.
High protein diets, such as the Atkin's diet or the Paleo diet, have been popular weight-loss programs in recent years. A high-protein diet has created concern, however, because it is high in meat, animal products, and saturated fat. Numerous studies have shown that a high meat intake is associated with increased risk for heart disease, cancer of the digestive tract, overall mortality, and acceleration of chronic kidney disease, kidney stones, and osteoporosis.1

Criteria for entry into the study were liver biopsy demonstrating ⩾grade 1 steatosis, overweight or obese (BMI ⩾25 kg/m2 in Caucasians and ⩾23 kg/m2 in Asians) or weight gain of >10% of usual body weight within 12 months, and alcohol consumption <10 g/day.

The effects of ketone drinks on endogenous insulin secretion are unclear. Whilst the small increase in plasma insulin after KE and KS drinks may have been due to the small quantity of dextrose in the diluent, it has been proposed that ketones could potentiate or even stimulate insulin secretion. Isolated pancreatic islets secreted insulin when stimulated by ketones at glucose concentrations of >5 mM (Biden and Taylor, 1983), and small amounts of insulin are secreted in vivo following exposure to exogenous ketones in animals (Madison et al., 1964; Miles et al., 1981). In response to an intra-venous 10 mM glucose clamp, ketone ester drinks increased glucose uptake and plasma insulin (Holdsworth et al., 2017). The increases in insulin with ketone drinks taken whilst fasted were small compared to the increases seen when the ketone ester drink was consumed with a meal and with consumption of a dextrose drink. Furthermore, the lack of difference in peak plasma insulin between the two latter conditions indicates that nutritional ketosis did not inhibit or increase normal carbohydrate induced insulin production.

There's no way to directly measure how your metabolic rate changes from workout to workout, but a good gauge is how you sweat. As you burn calories at a higher rate, you'll begin to perspire sooner into your workout and more than usual. It's a simple formula to follow: Keep your metabolic rate up and lose weight; let it drop and body fat increases.

Sugar comes in many forms. The label might not say “sugar”, but if the words end in ‘ose’, it means it is still a sugar. A “healthy” breakfast of cereal, yoghurt, and fruit with a glass of orange juice can contain up to 14 teaspoons of sugar – the recommended daily amount is 7 teaspoons.

You don’t say how long it has been since you have “cheated”? Have you gone more than one week without wheat and packaged foods? Are you eating vegetables, meats AND fats? If you have strictly been making sure your are not eating wheat or junk (high sugar/carb, or processed food, or eating 10,000 calories) and that you are following the Doc’s recommendations, I would almost think you have a medical issue (allergy to something new in your diet??)- 15 lbs in 2mos (saying you ate good after the first two months) is a lot of weight to gain in such a short period.
Weight loss is difficult problem for many people. One culprit that often sabotages diet plans is sugar. This sweet substance is high in calories and low in nutrition. Sugar is also craved by lots of folks, making it difficult to enjoy eating without it. Although you do not need to eliminate sugar completely to lose weight, it is necessary to cut down on its consumption. The American Heart Association reports that men should have no more than 150 calories, or 9 teaspoons, of added sugar per day, and women should have no more than 100 calories or 6 teaspoons per day. Consuming excess added sugar can increase your risk of dying from heart disease. There are some strategies to make lowering your sugar consumption easier, thereby increasing your likelihood of dropping pounds.
If you are consistently exercising, then your muscles need fuel and your liver will help send fuel in their direction. If you are too inactive then your liver is confronted by a huge problem of what to do with all the excess fat and sugar. Your white adipose tissue is bursting at the seams, your liver itself is drowning in surplus, and no place in your body needs all that is available. To cope with this situation your liver turns on a last ditch backup mechanism by dumping the excess calories back into your digestive tract. This process is incredibly inefficient and is the antithesis of what your body is designed to do, which is to efficiently extract calories from food and get them into your body. Never in your body's wildest genetic dreams, did it think it would need to cope with being poisoned by too much food.
Human's ability to produce and oxidize ketone bodies arguably evolved to enhance survival during starvation by providing an energy source for the brain and slowing the breakdown of carbohydrate and protein stores (Owen et al., 1967; Sato et al., 1995; Marshall, 2010). The brain is normally reliant on carbohydrate as a substrate, being less able to metabolize lipids, despite adipose tissue representing a far larger energy store than muscle and liver glycogen. Therefore, during starvation, lipids are used for hepatic ketogenesis and, via ketone bodies, lipids sustain the brain. Endogenous production of the ketone bodies, d-β-hydroxybutyrate (βHB) and acetoacetate (AcAc), increases slowly, driven by interactions between macronutrient availability (i.e., low glucose and high free fatty acids) and hormonal signaling (i.e., low insulin, high glucagon and cortisol). Produced continuously under physiological conditions, blood ketone concentrations increase during starvation (Cahill, 1970), when consuming a “ketogenic” (low carbohydrate, high-fat) diet (Gilbert et al., 2000) or following prolonged exercise (Koeslag et al., 1980).
On a ketogenic diet, your entire body switches its fuel supply to run mostly on fat, burning fat 24-7. When insulin levels become very low, fat burning can increase dramatically. It becomes easier to access your fat stores to burn them off. This is great if you’re trying to lose weight, but there are also other less obvious benefits, such as less hunger and a steady supply of energy. This may help keep you alert and focused.
Calories. To lose weight, you need to burn more calories than you take in. There are several ways to reduce the number of calories you eat, including reducing portion sizes; limiting added sugars and saturated and trans fats; and choosing fruits, vegetables, whole grains, lean proteins, and healthy fats instead of processed foods. And keep in mind that as you age, you may need to eat even fewer calories. This is because the amount of muscle you have tends to decrease as you get older. Your muscle mass affects how many calories you need because muscle tissue burns calories, even at rest. So having less muscle decreases your calorie needs by decreasing your basal metabolic rate, while having more muscle increases your calorie needs by increasing your basal metabolic rate.
