Ketogenic diets have been successfully used to treat diseases that have an underlying metabolic component, effectively decreasing seizures in recalcitrant pediatric epilepsy (Kossoff et al., 2003), lowering blood glucose concentrations in type 2 diabetes mellitus (Feinman et al., 2015) and aiding weight-loss (Bueno et al., 2013). Emerging evidence supports several clinical uses of ketogenic diets, for example in neurodegenerative diseases (Vanitallie et al., 2005), specific genetic disorders of metabolism (Veech, 2004) and as an adjunct to cancer therapy (Nebeling et al., 1995). Ketone bodies themselves may underlie the efficacy of the ketogenic diet, either through their role as a respiratory fuel, by altering the use of carbohydrate, protein and lipids (Thompson and Wu, 1991; Cox et al., 2016), or through other extra- and intracellular signaling effects (Newman and Verdin, 2014). The difference in peak blood d-βHB concentrations between matched amounts of βHB as ester or salts arose because the salt contained l-βHB, as the blood concentrations of d- plus l-βHB isoforms were similar for both compounds. It is unclear if kinetic parameters of KE and KS drinks would be similar if matched d-βHB were taken in the drinks. Unlike d-βHB, blood l-βHB remained elevated for at least 8 h following the drink, suggesting an overall lower rate of metabolism of l-βHB as urinary elimination of l-βHB was in proportion to plasma concentration. Despite similar concentrations of total βHB, breath acetone was ~50% lower following KS drinks compared to KE, suggesting fundamental differences in the metabolic fates of D- and L-βHB. These findings support both previous hypotheses (Veech and King, 2016) and experimental work in rats (Webber and Edmond, 1977), which suggested that the l-isoform was less readily oxidized than the d-isoform, and is processed via different pathways, perhaps in different cellular compartments. It seems that l-βHB is not a major oxidative fuel at rest, and may accumulate with repeated KS drinks. However, the putative signaling role of l-βHB in humans remains unclear. In rodent cardiomyocytes, l-βHB acts as a signal that modulates the metabolism of d-βHB and glucose, Tsai et al. (2006) although no differences in blood glucose were seen here. Furthermore, L-βHB can act as a cellular antioxidant, although to a lesser extent than D-βHB (Haces et al., 2008).

Retinol-binding protein-4 (RBP-4) and adiponectin are two important adipocytokines that may relate to insulin resistance and dyslipidemia in metabolic syndrome (9,10). Weight loss has been shown to lower plasma RBP-4 and elevate adiponectin levels (11,12). These effects may account for improvement in dyslipidemia with weight loss by regulating hepatic output and catabolism of VLDL, with associated remodeling of both LDL and HDL particles. The extent to which both RBP-4 and adiponectin are associated with lipoprotein kinetics after weight loss in obesity remains to be clarified. Moreover, the remodeling of these lipoprotein particles is also regulated by cholesteryl ester transfer protein (CETP) and phospholipid transfer protein (PLTP) (13). However, the effect of weight loss on CETP and PLTP activities (14) and the corresponding impact on LDL and HDL metabolism are also unclear.

Serum lipoproteins, body composition, and adipose cholesterol contents of six obese women were studied during and after major weight loss by very-low-calorie diets (VLCDs). Subjects started at 168 +/- 11% of ideal body weight, lost 30.3 +/- 3.7 kg in 5-7 mo, followed by 2+ mo in weight maintenance. Serum cholesterol fell from a prediet (baseline) value of 5.49 +/- 0.32 to 3.62 +/- 0.31 mmol/L (P less than 0.01) after 1-2 mo of VLCDs (nadir), after which it rose to 5.95 +/- 0.36 mmol/L (peak, P less than 0.01 compared with nadir and baseline) as weight loss continued. With weight maintenance, serum cholesterol fell to 4.92 +/- 0.34 mmol/L (P less than 0.05 compared with peak). Adipose cholesterol content did not change in peripheral (arm and leg) biopsy sites but rose significantly in abdominal adipose tissue with weight loss. We conclude that major weight loss was associated with a late rise in serum cholesterol, possibly from mobilization of adipose cholesterol stores, which resolved when weight loss ceased.

All analyses were performed using SPSS version 15 (SPSS, Chicago, IL). Skewed data were log-transformed where appropriate. Treatment effects of the weight loss group relative to the weight maintenance group were analyzed using general linear modeling with adjustment for the dependent variable at baseline (i.e., end of study variable = baseline variable + treatment group + constant). Statistical significance was defined as P < 0.05.
Table 3 shows the kinetic indexes for VLDL, LDL, and HDL metabolism in the two groups. There were no significant group differences in lipoprotein kinetics at baseline. As before (13), weight loss significantly decreased the pool size (−41%, P = 0.007), concentration (−47%, P = 0.003), and production rate (−47%, P < 0.05) of VLDL apoB-100 but did not change VLDL apoB-100 FCR. There was a significant decrease (P < 0.05) in the weight loss group in the plasma LDL apoB-100 concentration (−24%) and pool size (−23%), as well as a significant increase in the LDL apoB-100 FCR (+27%), but no change in the LDL apoB-100 production rate. Weight loss was also associated with an increase in the percent conversion of VLDL apoB-100 to LDL apoB-100 (+23%, P < 0.01), and this increase was chiefly attributed to channelling via IDL (+16%, P = 0.06). The increase in LDL apoB-100 FCR was significantly correlated with the decrease in the pool size of LDL apoB-100 (r = −0.60, P < 0.01). Compared with weight maintenance, weight loss decreased HDL apoA-I production (−13%, P < 0.05) and FCR (−13%, P = 0.02), with no significant changes in the plasma concentration or pool size of HDL apoA-I. The changes in HDL apoA-I FCR and production rate were highly correlated (r = 0.72, P < 0.001). However, the changes in LDL and HDL FCR with weight loss were not statistically correlated.

“If you’re going to use lifestyle to lower your cholesterol, you have to do it regularly. You can’t just do it for a few months and then quit,” says Dr. Goldberg. Cutting your intake of obvious sources of sugar is the first step in reducing how many calories you consume. Eliminate soda, fruit-flavored drinks, candy, cake, cookies, brownies, ice cream and other desserts from your daily diet. You don't have to completely avoid sweet treats, but strictly limiting your intake is one way to reduce how many calories you consume from added sugar so that you're able to reach your weight-loss goals.
Other issues relating to digestion, toxins, metabolic flu, and germ gangs, as explained in articles #2 through #5 in this series are also important to consider. If you have problems with any of these topics, which is typical when weight is not readily responding to better diet and exercise, then these issues should be addressed to help engage the process of weight loss. Strength training is the form of exercise that can have an influence on your resting metabolic rate. Increasing your muscle mass increases your metabolism. In fact, that's why your metabolism winds down as you get older: as you age, you lose muscle. While muscle and fat weigh exactly the same on the scale, muscle is compact tissue and it is also active tissue — it burns calories.

TIP: Try replacing cow's milk with almond milk and choose grass-fed products. Instead of meat, use legumes like black beans or chickpeas as well as root vegetables like carrots and beets. Mushrooms are a great meat substitute since they can have a similar consistency, and they're both flavorful and filling. Instead of eating meals where meat is the main dish, make soups or stews or chili. With these dishes it is easy to cut back on some meat and throw in more vegetables instead.

The ratio of fat to muscle in the body also affects metabolic rate. Weight, or body composition, is made up of fat, muscle, bone and water. Muscle is more metabolically active than fat. In other words, it burns more calories. When you lose weight, you lose both fat and muscle, unless you are doing something to preserve the muscle mass. Losing calorie-burning lean muscle mass slows your metabolism.

At enrolment, BMI had a strong negative correlation with the HRQL physical component score (rs = −0.48, p = 0.004) and was also negatively correlated with four SF-36 health domains, including physical functioning (r = −0.54, p = 0.001), general health (r = −0.40, p = 0.02), social functioning (r = −0.40, p = 0.02), and bodily pain (r = −0.40, p = 0.03). Compared with population norms,23 both the PCS and MCS were significantly decreased (p = 0.0003 and p = 0.0007, respectively) (fig 4A, B) and seven of the eight SF-36 health domains scored significantly lower in patients with chronic liver disease at t = 0. After the initial three month intervention, PCS and MCS significantly increased (p<0.0001 and p = 0.004, respectively) (fig 4A, B) and all but one health domain were comparable with population norms. In patients who maintained weight at t = 15, both PCS and MCS remained significantly higher than enrolment scores (p = 0.005 and p = 0.003, respectively). In contrast, in patients who regained weight, PCS and MCS scores decreased after 15 months and were no different to those at enrolment (p = 0.12 and p = 0.06, respectively) (fig 4A, B). Although mean PCS score was higher at t = 0 in patients who maintained weight, this did not reach statistical significance (p = 0.10). There was no association between fibrosis score and quality of life in patients with chronic liver disease.
