A survey in 2005 of 88 paediatric neurologists in the US found that 36% regularly prescribed the diet after three or more drugs had failed, 24% occasionally prescribed the diet as a last resort, 24% had only prescribed the diet in a few rare cases, and 16% had never prescribed the diet. Several possible explanations exist for this gap between evidence and clinical practice.[34] One major factor may be the lack of adequately trained dietitians, who are needed to administer a ketogenic diet programme.[31]
Russel Wilder first used the ketogenic diet to treat epilepsy in 1921. He also coined the term "ketogenic diet." For almost a decade, the ketogenic diet enjoyed a place in the medical world as a therapeutic diet for pediatric epilepsy and was widely used until its popularity ceased with the introduction of antiepileptic agents. Another common mistake is to eat the same or similar every day regardless of the riding you are doing. For example, a big bowl of porridge is brilliant on the morning of a big ride but, if you’re not cycling that day or not doing a session until later in the day, it is not necessary. A less carbohydrate and calorie heavy breakfast, such as an omelette or yoghurt with fruit, would be more appropriate. Conversely, if you are riding hard and the quality of the session is your priority, it is important that you take on sufficient carbohydrates. At the same time as you plan your training, plan your diet so that it is appropriate to the riding you are doing.
