Visceral fat develops in the abdomen below the muscle layer. Tumescent liposuction works on the subcutaneous fat, or the fat directly under skin. This procedure does not go below the muscle where visceral fat is located. Tumescent liposuction (as with all liposuction procedures) is good for only the fat that is above the muscle and easily reachable. Recently, many of my patients have been asking about a ketogenic diet. Is a ketogenic diet safe? Would you recommend it? Despite the recent hype, a ketogenic diet is not something new. In medicine, we have been using it for almost 100 years to treat drug-resistant epilepsy, especially in children. In the 1970s, Dr. Atkins popularized his very-low-carbohydrate diet for weight loss that began with a very strict two-week ketogenic phase. Over the years, other fad diets incorporated a similar approach for weight loss.


Ultrasound-assisted liposuction. During this procedure the surgeon inserts a special cannula through small "access" incisions. The cannula emits sound waves to help break up the fat, making it easier to vacuum. Vaser-assisted liposuction is the third-generation version of ultrasound assisted liposuction technology. The cannula used for Vaser-assisted liposuction emits gentler sound waves to break up and then remove fat.


Liposuction is performed under general anesthesia in an accredited surgical facility. It is considered an outpatient procedure. On average, each body part (i.e. tummy, thighs, etc.) will take approximately 30 minutes to complete. After surgery, patients will wear a compression garment on the treated areas. It takes about a full week for the bruising and swelling to dissipate, and can take several months for the final results to appear. Liposuction will produce scars where the cannula incision was made, but these are usually small and will fade over time.
During the ultrasonic tumescent liposuction procedure, fat is liquefied by sound waves prior to removing it with suction. Before surgery, I inject the parts of the body to be treated with a tumescent fluid containing local anesthetic and epinephrine.  In addition to the effects mentioned above with the conventional liposuction procedure, this liquid causes deposits of fat to swell so that they can be removed smoothly, with minimal trauma to the body’s other tissues.  As the sound waves penetrate the fat deposits, the fat melts, easing the removal process.  This is especially useful in patients who have scar from prior liposuction and male patients with more fibrous fatty deposits.  The use of ultrasonic sound waves can also help the skin tighten around the treated area.  Ultrasonic liposuction can lead to more unpredictable results after surgery, increased sensory alterations and increased potential contour abnormalities.  I generally reserve ultrasound liposuction for redo liposuction patients who have more scar tissue and for male patients with more fibrous fat.
Recovery time differs for each patient, but in general, you can return to work two to four days after your liposuction treatment. The exact time will depend on the area you had treated and your body's natural healing process. If you have an extremely physical job, you may need to take more time off; doctors recommend that patients wait at least three weeks after liposuction before engaging in aerobic exercise.
