But in June 2018, the authors took the rare step of retracting the original study in the New England Journal of Medicine based on flaws in how the original study was conducted. It turns out that about 15 percent of the people in the study weren’t actually placed in a particular group randomly — people with a family member also participating were placed in the same group; one clinic assigned everyone to the same group; and another study site didn’t properly use the randomization table.

Nuts are another Mediterranean diet staple. Grabbing a handful, whether that's almonds, cashews, or pistachios, can make for a satisfying, on-the-go snack. One study in Nutrition Journal found that if people replaced their standard snack (cookies, chips, crackers, snack mix, cereal bars) with almonds, their diets would be lower in empty calories, added sugar, and sodium. Recently, many people have begun to question the usefulness of BMI as a general indicator. A sizable amount of people who may be overweight or even obese may be classified as such even though they have a high amount of muscle. Conversely, people with a “healthy” or low BMI may have a high level of body fat and actually be at a greater risk for developing health problems. [5, 6]
One interesting finding of this eating plan is that it dispels the myth that people with or at risk for heart disease must eat a low fat diet. Although it does matter which types of fats are chosen, the percentage of calories from fat is less of an issue. The PREDIMED study, a primary prevention trial including thousands of people with diabetes or other risk factors for heart disease found that a Mediterranean diet supplemented with extra virgin olive oil or nuts and without any fat and calorie restrictions reduced the rates of death from stroke by roughly 30%. [6] Most dietary fats were healthy fats, such as those from fatty fish, olive oil, and nuts, but total fat intake was generous at 39-42% of total daily calories, much higher than the 20-35% fat guideline as stated by the Institute of Medicine. [7]  Risk of type 2 diabetes was also reduced in the PREDIMED trial. [8]
However, this diet is gaining considerable attention as a potential weight-loss strategy due to the low-carb diet craze, which started in the 1970s with the Atkins diet (a very low-carbohydrate, high-protein diet, which was a commercial success and popularized low-carb diets to a new level). Today, other low-carb diets including the Paleo, South Beach, and Dukan diets are all high in protein but moderate in fat. In contrast, the ketogenic diet is distinctive for its exceptionally high-fat content, typically 70% to 80%, though with only a moderate intake of protein.
A diet high in fresh plant foods and healthy fats seems to be the winning combination for longevity. In 2017, Drs. Kevin Hall and Juen Guo published a meta-analysis of controlled feeding studies that compared diets of equal calorie and protein content with variations in carbohydrate and fat content. A 2013 study conducted by the German Research Center for Food Chemistry indicates that even just smelling EVOO may lead to greater feelings of fullness: when the scent was added to foods via an aromatic extract, it lowered the number of calories consumed by study participants, and improved blood sugar response. Additionally, compared to other oils and fats, when EVOO was added to yogurt, the group that had eaten the yogurt enriched with olive oil showed the largest increases in blood levels of serotonin, a hormone associated with satiety.
And monounsaturated fat isn't the only thing olive oil has going for it nutritionally. Some olive oils come with phytonutrients that may offer their own disease protection benefits (still, it's not clear whether most of us can take in enough of these phytonutrients without going overboard on olive oil, says Joyce Nettleton, DSc, RD, researcher and editor of the PUFA Newsletter).
